The CMP accepts referrals from the JSACC, local schools, JPPO, as well as family and youth themselves. The CMP is a program of longer term services based upon assessing the needs of the client and the client’s family, when appropriate and arranging, coordinating, monitoring, evaluating & advocating for a package of multiple services to meet the client’s complex needs. Typically clients are in the CMP between 2 to 6 months and sometimes longer as needed.
